ORDERING INSTRUCTIONS
Just indicate the number of specific style/color t-shirts, tanks, sweatshirts you wish to purchase. The order form is interactive so you can complete it, include the total dollar amount, print it out and mail it with your payment.
The Men's tees [Hanes Beefy], Unisex tanks, and the Sweat crewnecks can be ordered for EITHER men or women. There is also a Women’s only tee & tank, if preferred.
If you wish to order a size larger than 2X, please indicate the size under 'TYPE' and we will see about ordering the style/color of shirt you have indicated. If the style/color is not available in what you request, we will request the next closest style/color in your size.
Please be advised: SHIRT/SWEATSHIRT SIZES LARGER THAN 2X WILL INCUR AN ADDITIONAL $2 CHARGE.
PLEASE COMPLETE THE BOTTOM OF THE ORDER FORM NOTING TOTAL AMOUNT DUE.
INCLUDE YOUR NAME AND A CONTACT TELEPHONE NUMBER ON THE BOTTOM OF THE ORDER FORM.
Print off the order form.
MAKE CHECK PAYABLE TO ‘BLPH’ w/ a memo note on the check - ‘club shirts.’

AGAIN, please have your order mailed so that it arrives NO LATER THAN WEDNESDAY, October 11, 2006.
Also, as an extra safety check, please follow-up with an email to Arlene -- phinzup@earthlink.net - that you are mailing an order and give her the number of t-shirts you will be purchasing.
Shirts will arrive prior to the end of October and members will be notified when the shirts are in and ready for delivery.
